1. What this app is

This is a digital version of a counselling intake, informed consent, progress note, and termination summary form set, used by counsellors to record client information during the counselling process. It runs entirely in your web browser.

2. What information is collected

When a counsellor fills in the form, the following types of information may be entered:

This information is entered directly by the counsellor (and, where relevant, the client) and is not collected from any other source.

3. Where this information is stored

This app does not operate its own server or database. Data you enter is stored in two places only:

4. Google Drive access (drive.file scope)

This app uses Google's drive.file permission scope, which is a limited, non-sensitive scope. In practice this means:

5. Who is responsible for the data (data controller)

Because each signed-in counsellor's records are saved into that counsellor's own Google Drive, each counsellor (or their employing clinic/organization) is the controller of the client data they enter and save — not the developer of this form. [Intake] is responsible for ensuring client data collected through this form is handled in line with applicable data protection and clinical record-keeping obligations in their jurisdiction, and for obtaining any consent from clients required for that record-keeping.

6. Cookies and tracking

This app does not use analytics, advertising, or tracking cookies. Google's sign-in process itself may set cookies required for authentication, governed by Google's own Privacy Policy.

7. Data retention and deletion

Records remain in a counsellor's Google Drive until that counsellor deletes them, exactly like any other file in their Drive. Locally autosaved drafts can be cleared at any time using the "Clear all" button in the form, which removes the data from that device's browser storage.

8. Security

The form is served over HTTPS. Sign-in is handled entirely by Google's own authentication system (Google Identity Services); this app never sees or stores your Google password. Saved records are protected by the same security Google applies to the rest of your Google Drive account.

10. Changes to this policy

This policy may be updated from time to time. Continued use of the form after changes are posted constitutes acceptance of the updated policy.
